Dance Classes of all types can involve risk of personal injury. THE FARM takes all reasonable care in the conduct of the classes and activities that are a part of the Bare Bones program and will accept no responsibility for injury or loss caused during the program or while participants are in The Farm Studio. All artists and participants attending a class, activity, or session of the program are responsible for ensuring that they are physically and mentally fit for the program and must at all times take care of their own personal safety during classes. The participant, and where the participant is under 18 years of age, the participant’s guardian warrants and certifies that:

● The participant is fully responsible for the safety, management, and care of their bodies during the program;

● The participant is not currently injured or currently under any medical or rehabilitation supervision for any old or re-occurring physical injuries or health conditions that might be affected through the course of this activity;

● The participant will follow safety instructions and will immediately fully inform each teacher of the program ofany physical difficulties or injuries that occur during the class as THE FARM does not accept liability for personal injury;

● the participant, in a show of trust and understanding, authorizes THE FARM to seek appropriate medical

attention in the event that they are injured. They also agree to pay all medical and ambulance costs that may be incurred as a result of their actions while participating in the program;

● The participant will complete an incident report within two hours of an injury or incident occurring. The teacher can provide this report form.

● The participant agrees to indemnify THE FARM and its directors, officers, employees, and teachers against all costs incurred, expenses and all liabilities whatsoever arising as a result of their actions or trainer instructions during all activities undertaken with the program and all performances;

● where the participant has an exception to any of these warranties, the participant will personally speak to an

administrative staff member prior to participating in the program;

● the participant understands that the classes may be filmed, photographed or documented in some way by THE FARM and release THE FARM to use their images in their advertising, archiving, publicity, marketing and other related activities
